Health Related Quality of Life Among Patients With Advanced Heart Failure: A Single Center Cross Sectional Study

OBJECTIVES: Heart failure (HF) is a chronic disease that can significantly affect patient’s health related quality of life (HRQOL). The objective of the study is to assess HRQOL among patients with advanced HF in Saudi Arabia.

METHODS: This was a cross-sectional study conducted at King Saud University Medical City from June 2023 - March 2024. HF patients with NYHA classification III and IV were included in the study. Clinical and demographic data were collected from patients’ medical records. Patients were interviewed face-to-face using Short-Form Health Survey (SF-36) questionnaire. Descriptive and inferential statistics were used.

RESULTS: A total of 150 HF patients completed the survey and included in the analysis (mean age: 54 years, 64.5% male, mean EF 28%). Overall, the patients’ scores were low for all quality of life domains. Physical functioning and role limitations due to physical health domains had mean scores (32, 34) respectively, while the highest scores were reported in emotional wellbeing and social functioning domains (58, 56) respectively. The overall Physical Component Summary (PCS) and Mental Component Summary (MCS) mean scores were (40.3±3, 52.2±5). The PCS and MCS scores varied significantly (p<0.001) between stage III and IV (45, 34) and (55, 48) respectively. In addition, the regression analysis found that every reduction in EF by 1% will significantly reduce the PCS and MCS scores by 3 and 2 points respectively (p<0.001).

CONCLUSIONS: The majority of HF had poor HRQOL, and it decreases as NYHA functional class worsens. Specific interventions targeting improving HRQL continue to be urgently needed.
