Epidemiology and Patient Characteristics of Pemphigus Vulgaris in UK: A Retrospective Cohort Study Using Clinical Practice Research Datalink (CPRD) and Hospital Episode Statistics (HES)

OBJECTIVES: Pemphigus diseases are life-threatening, chronic, autoimmune blistering diseases. This research aims to explore epidemiology of pemphigus vulgaris (PV) and patient’s characteristics in UK.
METHODS: This retrospective cohort study included patients with PV using CPRD Aurum and HES data from January 2010 to December 2020. Eligible patients require at least two diagnosis for PV (ICD-10: L10.0 or relevant SNOMED code), aged ≥18 years, have no diagnosis of any non-vulgaris form of pemphigus (ICD-10 code L10.1, L10.2, L10.3, L10.4, L10.5, L10.8, L10.9) in HES or relevant SNOMED code in the 1 year pre-index period, and at least 12 months follow-up period.
We considered the index date as the earliest diagnosis code in the study period. Patients were considered as incidence if no previous diagnosis was recorded at least 12 months before the index date.RESULTS: In CPRD-HES database, we identified 379 prevalent patients with PV and 339 incident patients. At the index date, most patients were female (52.8%), mean (SD) age was 61.37 years (17.44), median years of follow up per patient were 3.11, predominance of white ethnicity (57.5%), mean (SD) body mass index was 27.65 (6,81) and 19.2% have a deprivation index score at 5. Only 8.3% patients were current smokers and less than 3% had heavy alcohol consumption . The main comorbidities are malignancy (55.8%), rheumatoid arthritis (20.6%), diabetes type 2 (19,8%), depression (15.3%) and osteoporosis (14.7%).
Overall PV incidence was stable for the period 2010 to 2020, with a range of 3.1 to 4.2 cases per million. The prevalence increased steadily over the period from 38 in 2010 to 249 in 2020 (4.5 to 22.9 per million).CONCLUSIONS: We observed an increase in the absolute incidence of pemphigus per year. However, this absolute increase in incidence, was not reflected in the incidence per million which remained relatively flat over the study period.
