Clinical Manifestations and Health Care Resource Utilization of Eosinophilic Granulomatosis with Polyangiitis (EGPA) in China: A Cross-Sectional Multi-Center Physician Survey Study

Objectives: Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare, inflammatory-mediated, systemic necrotizing vasculitis affecting small-to-medium-sized vessels, which can lead to severe organ damage. This study aims to describe the clinical manifestations of EPGA patients in China and associated health care resource utilization (HCRU). Methods: In Aug 2021, 16 physicians from 14 top-tier hospitals within China were recruited, among which 15 physicians completed the online survey, including 4 respiratory physicians and 11 Rheumatologists. Aggregated data based on 243 EGPA patients were reported by physicians. Median and proportion were used to summarize results. Results: Average disease duration of EGPA in China was 14.5 years. Lung (80%), respiratory tract (66.7%), and sino-nasal (53.3%) were the most frequently reported organs involved at diagnosis. Among the median 8.5 general EGPA patients treated by each physician in the past 12 months, 3.0 were with relapsed or refractory disease (R/R) and 3.0 were corticosteroid-dependent. Compared to general patients, corticosteroid-dependent patients had a lower remission rate (57.1% vs. 85.0%) and a higher relapse rate (60.0% vs. 40.0%); both R/R patients (40.0%) and corticosteroid-dependent patients (25.0%) had a higher 10-year mortality rate than general patients (10.0%). Among three groups, R/R patients had the highest HCRU, including 2.0 hospitalization admissions in general ward, 10.0 outpatient visits and 2.0 ER visits per year, and length of stay per admission of 15.0 days in general ward and 20.0 days in ICU. R/R patients (20,000.0 CNY) and corticosteroid-dependent patients (15,000.0 CNY) had a higher cost per hospitalization admission in general ward than general patients (12,000.0 CNY), while the disposable income per capital in China in 2020 was 32,189 CNY. Conclusions: EGPA is associated with a high clinical burden due to organ involvement at diagnosis and poor prognosis, which also lead to substantial economic impact, particularly among R/R patients and corticosteroid-dependent patients.
