The Dental Care Needs and the Government's Intervention Service in People With Special Needs in Taiwan

OBJECTIVES: The people with disabilities usually gets less dental care then ordinary population, and it get worse with increasing age. The study investigated the dental care and the government's intervention service in people with specific disabilities (PSDS).
METHODS: This cross-section study was conducted to analyze the insurance reimbursement of dental services in 2016-2019 National health insurance research database and disability database. It stratified it at different disability type and severity, the hospital levels to examine the utilization of oral care of PSDS and evaluated the effectiveness of related policies.
RESULTS: The utilization of dental care of PSDS for treatment of dental disease and get preventive care was 33.5% to 34.6% in 2016-2019. The rate of PSDS seeking dental care across other counties and cities was about 40 to 50% in 2016-2019. The utilization of dental auxiliary medical treatment by PSDS is low from 2016 to 2019. The utilization of general anesthesia is only about 0.2% to 0.3% per year. The utilization of the protection sexual limb restraint is only about 1.1% to 1.3% every year, and the utilization of sedation and anesthesia is less than 0.1%. The utilization of dental care with PSDS was the lowest in vegetative patients that was about 12% to 14% in 2016-2019.
CONCLUSIONS: The results of this study, it is the same as the Italian experience. Dentists may be less willing to provide dental care services to people with cognitive impairment and poor coordination ability. The government should provide more training program and incentives so that dentists are willing and not afraid to provide dental care services for the elderly and those with more severe disabilities of PSDS.
