Impact of Phase Angle in Quality of Life and Mortality in Male With Head and Neck Cancer

INTRODUCTION: The quality of life (QoL) of patients with head and neck cancer (H&NC) is related to their nutritional status at the time of diagnosis. In this report health-related quality of life, survival and risk of death and their relationship to phase angle (PA) were studied.

OBJECTIVES: To determine the role of phase angle in survival and QoL in a men population with H&N cancer

METHODS: The selected subjects were divided into two groups: men with PA < 4.47° and men with PA ≥ 4.47°. Both of them were followed up for at least two years. We evaluated survival and quality of life

RESULTS: Sample studied of 107 patients with a mean age of 65.4 years. Patients with a PA ≥ 4.47 had an ≈30% higher probability of survival at two years with a better quality of life compared to patients with a PA< 4.47°. Statistical differences were found in the functional and symptom scales, with lower functional scores and higher symptom scores in patients with low PA. In QLQ-C30 the observed differences were Global Health State/QoL (mean 63 versus 76; p=0.006), physical (64.9 versus 86.9, p=0.000) and role (64.5 versus 82.7, p=0.008) functioning, fatigue, pain and loss of appetite, in QLQ H&N35: deglutition, senses problems, troubles with eating social, less sexuality, opening mouth, dry mouth, sticky saliva, felt ill, pain killer, nutritional supplements and feeding tube, between PA <4.47 and ≥4.47 PA all with p<0.05. Median survival was 21.8 months for those with PA <4.47° versus 34.3 months for those with PA ≥ 4.47° (p<0.006). The percentage of deceased subjects (62.5%; p=0.005), with high mortality rates in patients with PA <4.47º. In patients with PA ≥4.47 the mortality rate was 33.3%.

CONCLUSIONS: Phase angle was the most crucial predictor of improved QoL, survival and a risk factor for death in male with H&NC.
