The Budget Impact of Introducing a Rapid Polymerase Chain Reaction Test for the Detection of Influenza and Respiratory Syncytial Virus in China

OBJECTIVES: To estimate the economic impact of introducing a rapid polymerase chain reaction (PCR) test to detect influenza and respiratory syncytial virus (RSV) from a fever clinic’s perspective and a patient's perspective in China.
METHODS: For a hypothetical one million patients, a budget impact model was developed to assess the difference in total healthcare costs and per patient per year (PPPY) costs before and after adding a rapid PCR test to detect influenza and RSV. The model evaluated testing fees (rapid PCR test, cFDA-PCR test, rapid antigen detection tests, immunochromatographic assay), hospitalization costs, influenza and RSV medication and supportive care costs, costs for other laboratory tests, labor costs of doctors, laboratory technicians and nurses, and other hospital operating costs for one year. Costs were estimated in 2022 Chinese yuan. The prevalence of influenza and RSV, length of influenza and RSV disease, percentage of RSV hospitalization, percentage of other laboratory tests during RSV hospitalization, and RSV hospitalization duration were obtained from published sources or expert opinions. In the base case, the market share of the rapid PCR test was assumed to be 60%, according to market research.
RESULTS: In the base case, all patients visiting the fever clinic are required to have influenza and RSV tests. The inclusion of the rapid PCR test decreased the healthcare budget by ¥0.22 billion from a fever clinic’s perspective and ¥139 PPPY from a patient’s perspective.The budget decrease was mainly due to fewer RSV hospitalizations, fewer other laboratory tests during RSV hospitalization, and shorter influenza and RSV disease duration. Results were the most sensitive to the percentage of RSV hospitalization.
CONCLUSIONS: The inclusion of a rapid PCR test to detect influenza and RSV offers an efficacious approach with a decrease in the healthcare budget from both a fever clinic’s perspective and a patient’s perspective in China.
