Significant Burden of Chronic Obstructive Pulmonary Disease (COPD) on Health Status and Well-Being Persists Among Patients Experiencing One Severe or Two or More Moderate Exacerbations of COPD (ECOPD) and Their Caregivers (CGs)

OBJECTIVES: Quantify the burden of COPD on patients and CGs.
METHODS: Cross-sectional design using electronic health records and survey data. Patients and their CG, were invited by their physicians, if patients met criteria of ≥40 years of age, COPD, 1-year history of ≥2 moderate or ≥1 severe ECOPD, using ≥1 inhaled maintenance medication, FEV1 ≥20% to <80%, no other significant pulmonary disease, and residing in Canada, Italy, or the United Kingdom (UK). Target sample size was 150 patient/CG dyads with enrichment of severe ECOPD. Data were summarized overall and by severity.
RESULTS: The study included 155 patients and 152 CGs (21% Canada, 41% Italy, 39% UK). Mean age 69 in patients, 59 in CGs; 29% of patients and 79% of CGs were female, 74% in urban areas, 18% of patients and 35% of CGs were employed, and 70% of CGs were the patient’s partner/spouse and 22% their children. 45% of patients had ≥1 severe ECOPD and 55% ≥2 moderate ECOPD. Calculated among all patients and using country-specific value sets, mean EQ-5D-5L was 0.684 Canada, 0.674 Italy, and 0.591 UK. The EuroQoL Health and Wellbeing short form yielded a mean UK index score of 0.608. Mean overall work impairment was 34% due to COPD in patients and 34% due to caregiving in CGs. 61% of patients scored high/very high (>20) on the Chronic Airway Assessment Test. 24% of CGs scored high burden (≥20) and 32% mild/moderate burden (10-19) on the Zarit-Burden Interview. Mean caregiving time was 4-5 hours/day and top tasks were managing the home (29%), activities of daily living (23%) and emotional support (15%). Burden measures were similar across ECOPD severity.
CONCLUSIONS: This study captured significant burden among patients with a 1-year history of one severe or ≥2 moderate ECOPD and their CGs, highlighting measures underreported in the literature.
