OBJECTIVES: The International consortium for outcomes measures (ICHOM) makes recommendations at an international level for implementation of patient centered outcome measures (PCOMs). This research aims to explore if the CTOMs along with the data set recommended in diabetic patients are feasible, the questionnaires are acceptable by the patients and propose a prospective interventional study based on the results.

METHODS: The French validity of recommended questionnaire by ICHOM was verified and adapted to local needs. Data was collected through two means: a patient self-questionnaire (Diabetes Distress Scale DDS; Patient Health questionnaire PHQ9; World Health Organization Well Being Scale WHO5; assessment of social and physical frailty (EPICES and FRIEDs scale) and compliance (Girerd’s scale)) and a health professional collection form including the data set as recommended. Exploratory Data Analysis algorithm was performed.

RESULTS: 42 diabetes patients were included in the study.

Diabetes-related distress is off 26%.

78 % of patients are at risk of minimal or mild depression; about 3% have a risk of moderately severe depression; less than 6% have a risk of severe depression; and almost 11% have a risk of moderate depression.

More than 24.4% of the patients Who-5 questionnaire present a risk of depression.

Over 50% of patients fall below the 30% precariousness threshold.

Physical frailty is over 28%.

Most patients present minimal problems with medication compliance.

The classification algorithm proposes a division of individuals into 3 clusters: cluster 1 not requiring additional healthcare support, cluster 2 with frail patient in need for emotional support, and cluster 3 including extremely distressed patients which require personalized healthcare support.

CONCLUSIONS: This approach could firstly help clinicians to identify obstacles to self-management of treatment, such as the feeling of being overwhelmed by diabetes or other life stressors and, secondly to guide the selection of interventions appropriate to patient needs.
