Budget Impact Analysis for the Use of Dupilumab in Patients with Severe Asthma in Colombia

OBJECTIVES: To determine the budget impact associated with financing dupilumab as a treatment of patients with severe asthma in Colombia from the healthcare system perspective. METHODS A five-year projective Budget Impact Model was developed from the healthcare system perspective including only direct medical costs. Two scenarios were compared in this study corresponding to 1) a scenario where the population is treated with either no biologic, or with one of the following biologics: omalizumab, mepolizumab, or benralizumab, and 2) an alternative market mix where the population is treated with either no biologic, or with omalizumab, mepolizumab, benralizumab or the addition of dupilumab The target population, clinical data for healthcare resources required, exacerbations rates, discontinuation rates for the alternatives and costs for the analysis were obtained from national public databases and published literature. Dosing for the alternatives was based upon approved prescribing information in Colombia. The market share of treatments projected for the five-year time frame in scenario 1 remains constant, and for scenario 2 dupilumab market share increases over time based upon local market evolution analysis. Costs included were treatment acquisition, treatment administration, and use of healthcare resources for exacerbation events management. RESULTS Using this analysis, a total population of 34,334 patients with severe asthma was estimated. The adoption of dupilumab into the Colombian healthcare system resulted in the following cost reductions USD $150,060, USD $629,366, USD $1.650 million, USD $2.511 million and USD $3.347 million for years 1 to 5 respectively. CONCLUSIONS: Based upon this Budget Impact Model, the use of Dupilumab to treat patients with severe asthma resulted in cost-savings to the healthcare system. From a financing perspective Dupilumab represents the best alternative to reduce costs to this patient population in the Colombian healthcare system.
