UKRAINIAN GUIDELINES FOR OSTEOARTHROSIS PHARMACOTHERAPY- RELEVANCE ANALYSIS AND RATIONALE FOR IMPROVEMENT

In Ukraine, the weight of osteoarthrosis (OA) in the overall structure of population morbidity is significant. It exceeds world trends and has significant annual increase. OBJECTIVES: The study goal was to conduct a comparative analysis of foreign and national guidelines for the pharmaceutical care of OA treatment. METHODS: The study examined the Osteoarthritis: care and management in adults. NICE guidelines, EULAR evidence based recommendations for the management of hand osteoarthritis, An algorithm recommendation for the management of knee osteoarthritis in Europe and internationally (ESCEO), systematic reviews of the Cochrane Library, Ukraine clinical guidelines Osteoarthrosis, protocol for healthcare of patients with OA, pharmacist protocol "Prevention and treatment of symptoms of osteoarthritis". The study used meta-analysis, content analysis, and generalization methods. RESULTS: The detailed comparative analysis of domestic OA treatment guidelines have shown that a clear, unified methodology of pharmaceutical care for patients with OA which meets the current ESCEO guidelines is set out only in pharmacist protocols. The protocol for the provision of medical care to patients with osteoarthrosis, includes a general list of groups of drugs that can be used for the treatment of OA, without considering the stage of disease. This list includes systemic enzyme therapy drugs and medicines that improve microcirculation, which are not mentioned by international recommendations. Clinical guidance "Osteoarthritis" is a significant shift towards harmonization of domestic OA treatment requirements with relevant international approaches, but it only provides general guidance and can not be considered as the standard of treatment and choice of pharmacotherapy. CONCLUSIONS: Thus, there is an urgent need to create national guidelines for the treatment of OA. Their development should include a modern pharmacoeconomic assessment based on the HTA methodology. Results of such assessment should substantiate the formation of the range of drugs for OA pharmacotherapy and their inclusion in the government reimbursement programs and medicines registries.
