TECHNOLOGICAL-ECONOMIC COMPARISON OF ENDODONTICS TECHNIQUE WITH FOCUS ON NIKL-TITAN SYSTEMS

OBJECTIVES: A general increase in the modernization of treatment practices using innovative technology has not avoided even the area of dentistry. This trend significantly contributes to improvement of the provided care. The main aim of this study was to evaluate the cost and clinical data of one of the most dynamically evolving stomatology procedure – endodontic root canal treatment with comparison to a plain tooth extraction METHODS: The chosen methods for achieving the objective were a cost analysis and a clinical-effectiveness analysis of individual interventions of root canal treatment and tooth extraction and subsequent cost-effectiveness analysis, a review of technological outputs for obtaining the effects of individual variants of endodontic nickel-titanium technology systems and a multicriteria decision method for choosing the most effective one. RESULTS: In the evaluation of the benefit of both dental procedures using the standardized Oral health impact profile questionnaire, the final effect value for endodontic treatment was 62.07% and for the extraction 16.6%. Based on the calculation of the minute-cost and direct material consumption, the cost of extraction treatment was calculated to 1,103 CZK and the one of endodontic treatment of single-channel tooth to 4,816 CZK. The results of the cost-effectiveness analysis were evaluated at 60 CZK per percentage of output at extraction and at 84 CZK per percentage of endodontic output. The value of incremental costs per percent of obtained endodontic treatment output relative to extraction was calculated to 93 CZK. Using multi-criteria decision-making, the fifth-generation of system was evaluated as the most efficient nickel-titanium technique, in particular One Shape. CONCLUSIONS: Endodontic treatment is more cost-effective than simple tooth extraction and the quality of life is significantly better after this treatment.
