EFFECT OF STATINS USAGE ON INCIDENCE OF PERIPHERAL NEUROPATHY AMONG PATIENTS WITH TYPE 2 DIABETES MELLITUS

OBJECTIVES: This study was designed to evaluate the association between statins use and peripheral neuropathy (PN) among patients with type 2 diabetes mellitus. Research question is that: Does the use of statins associated with higher or lesser peripheral neuropathy incidence among diabetic patients? METHODS: A retrospective cohort study was conducted at Pulau Pinang Hospital, in October 2016–2017. About RESULTS: Prescribed statins were 11.2% atorvastatin (20-80mg/daily), 5% lovastatin (10-60mg/daily), 1.6% pravastatin (10-40mg/daily), 1.8% rosuvastatin (10-20mg/daily) and 80.5% simvastatin (10-80mg/daily). About 129 (22.9%) cases from 564 statins users had PN. Only 30 (15.5%) subjects had peripheral neuropathy from 193 statins non-users. Chi-square test indicated a significant difference between statins users and non-users in the PN incidence (P-value: 0.001). Spearman’s analysis showed a positive correlation (r: 0.078, p-value: 0.031) between statins usage and PN incidence. Binary logistic regression was statistically significant for statins as a predictor of peripheral neuropathy incidence (r: 0.006, p-value: 0.027) among type 2 diabetic patients. The relative risk of peripheral neuropathy associated with statins usage is (RR: 1.47, 95% CI: 1.02 – 2.11). The excess relative risk is 47.1%. While the absolute risk is 7.3% and the number needed to harm is 14. CONCLUSIONS: In this study, we found a positive association between peripheral neuropathy and statins utilization. PN was higher among statins users than statins non-users’ cohort. Larger-sample size clinical-trial study is required to confirm the causal relationship between statins and PN.
