DEVELOPING STROKE SYSTEM CARE FOR LOW MIDDLE INCOME COUNTRIES .EGYPT MODEL

OBJECTIVES: In Egypt Stroke accounts for 6·4% of all deaths and thus ranks 3rd after heart disease &gastrointestinal with a crude prevalence rate of 963/100 000 inhabitants. The clinical characteristics of Egyptian stroke patients are generally similar to those in other populations. Exceptions may include the higher prevalence of vascular risk factors and a younger mean age of stroke patients (4, 18–20). Due to those facts stroke &its complications may be one of the major economic challenges facing Egyptian health sector. The objectives for this study is to evaluate establishing stroke care system based on economic value and multidisciplinary decision making for all parts of the system and it is impact on patient’s outcomes and utilization of resources in order to developing effective stroke policy . METHODS: Descriptive analysis of Data from patients aged (18 -70 years ) for the last 2 years including direct and indirect medical costs for conventional treatment was calculated for evaluation of utilization of health care resources. Including (cost of treatment, complications including physical therapy, hospitalization, outpatient’s costs, and rehabilitation ). SF36 questioner was used for evaluating impact of the program on quality of life multi dimensions sensitivity analysis was conducted. Level of statistical significance was defined at (p. <0.05). RESULTS: With age range 18–70 years, with mean age 62.19±12.65); 277 males (58.1%) with mean age 63.52±12.05 and 200 females (41.9%) with mean age 60.35±13.25 (P=0.007). total medical costs for Stroke complications was reduced with percentage (50%). Quality of life for patients who subjected to new stroke care system was increased with (75%). total health expenditures due to stoke management with reduced with (50%). CONCLUSIONS: Egypt model for establishing a stroke care system based on economic value and multidisciplinary decision making might have positive impact on patent’s outcomes and efficient use foe health care resources .
