COST-EFFECTIVENESS ANALYSIS OF SURFACTANT THERAPY FOR THE TREATMENT OF RESPIRATORY DISTRESS SYNDROME NEWBORN IN THE RUSSIAN FEDERATION

OBJECTIVES: To provide pharmacoeconomic evaluation of surfactant therapy for the treatment of respiratory distress syndrome newborn (RDSN) in the Russian Federation from hospital perspective. METHODS: We performed cost-effectiveness (CEA) analysis to assess Beractant (B) and Poractant alfa (PA) in the treatment of RDSN. Efficacy data was received from meta-analysis. Neonatal mortality was primary measurement of efficacy for cost-effectiveness analysis. Risks of patent ductus arteriosus, bronchopulmonary dysplasia, pneumothorax and requirement of additional dose of surfactants were used for cost analysis. Patient’s birth weight was taken into account when surfactant costs were calculated. Decision-tree model was developed; time horizon was equal to the duration of case of RDSN treatment. Exchange rate was 1 EURO = 70 RUR. RESULTS: Cochrane meta-analysis has shown that use of PA is associated with significant lower risk of patent ductus arteriosus, bronchopulmonary dysplasia, pneumothorax and requirement of additional dose compare to B in treatment of RDSN. Cost analysis resulted that expenditures for surfactants per patient depend on body weight and for patient with birth weight 1000-1500 gr were €615,6 for B and €550,2 for PA correspondently. Total medical costs of treatment of RDSN per patient were €4110,5 for B arm and €3768 for PA arm. Cost-effectiveness ratio (CER) was €4585 per saved life for PA and €5087 per saved life for B. One way sensitivity analysis has shown, that value of CER for PA remains lower compare to B, when cost of PA increased on 10%. CONCLUSIONS: Our results suggest, that use of PA in the treatment of RDSN in the Russian Federation is cost-saving and dominant from CEA perspective
