OBJECTIVES: This study performed pharmacoeconomic evaluation compared bariatric surgery program with the use of medication control in Thai morbidly obese diabetes mellitus type two patients.

METHODS: Pharmacoeconomic evaluation in this study will be explored throughout the modelling method including decision tree and Markov model. Cost-utility analysis was selected for this analysis. Collection of cost and effectiveness data is gathered by retrospective databased analysis and explicit systematic literature review. Target population of this study is DM type 2 patients with BMI

RESULTS: Bariatric surgery significantly improve clinical outcome including remission rate, HBA1C and BMI. The incremental cost per QALY of bariatric surgery compared with medication control is 26,907.76 THB/QALY which is below than Thailand’s threshold and can be considered as cost-effective option. The result from one-way sensitivity analysis suggested the robustness of the model.

CONCLUSIONS: Bariatric surgery in morbidly obese DM type two patients is cost effectiveness in Thai’s context. Therefore, it should assist the decision maker to adopt bariatric surgery into standard treatment of severely obese DM on Thailand.
