DESIGN AND VALIDATION OF KNOWLEDGE, ATTITUDE AND PRACTICE (KAP) QUESTIONNAIRE OF HEALTHCARE PROVIDERS TOWARDS STATINS UTILIZATION IN DIABETIC DYSLIPIDEMIA

OBJECTIVES: To develop and validate a questionnaire to detect the knowledge, attitude and practice of pharmacists and physicians towards statins use in the management of dyslipidemia among persons with type 2 diabetes mellitus in Pulau Pinang Hospital, Malaysia, 2016.

METHODS: The questionnaire was constructed based on an extensive literature review and experts’ opinion. Instrument design is performed through three- steps process, including determining content domain, sampling from content (item generation), instrument validation and construction. Ten experts rated the relevance and clarity, while ten healthcare providers rated the simplicity and ambiguity of the questionnaire items. The content validity and face validity of the questionnaire were estimated. Cronbach’s alpha and Pearson coefficient were used to estimate the internal consistency of the questions in the questionnaire and test-retest reliability of the questionnaire respectively among 35 subjects. The statistical analysis was conducted using IBM-SPSS version 22.

RESULTS: From a set of 42 items, content validity process identified three dimensions include knowledge (ten questions), attitude (six items), practice (ten questions). The overall content validity score (S-CVI) and the item content validity index (I-CVI) of the prepared questionnaire was 3.85 and 96.13% respectively. The internal consistency of the questionnaire measured using Cronbach’s alpha was found 0.783. The KAP scores for test and retest was (84.2 ± 2.95%) and (84.8 ± 2.79%) respectively. Paired Samples t-test revealed statistically non-significant difference among the means of the findings of the test and retest of KAP questionnaire (P-value: 0.539). The calculated Pearson correlation coefficient value of the test and retest scores was 0.947 (P-value <0.001).

CONCLUSIONS: The questionnaire is acceptable, and the I-CVI was high among all the four parameters of the content validity index. The prepared questionnaire showed high reliability and stability between KAP scores of the test and retest. The KAP questionnaire is simple and easy to use.
