THE JOHN KINGDON'S "POLICY STREAMS" APPROACH- A CONCEPTUAL FRAMEWORK FOR UNDERSTANDING A RISK SHARING AGREEMENT PILOT DESIGN BY FIRST TIME IN CHILE

Abstract: The Kingdom’s Policy Streams Approach provides a conceptual framework to understand policy formation process. The problem stream regards a public matter recognised as a problem by society. Policy stream concerns proposals for change coming from Academia or scientific organizations. And finally, politics stream composed by political issues. A conflict arises when these streams can’t be coupled given fragmented scenarios, and consequently a policy window will not be opened.  In Chile, high cost, infrequent and catastrophic conditions are not financially covered by health system. Recently, after social unrest a Fund for financial protection was enacted, persisting uncertainty about right payment-mechanisms for its implementation. In these circumstances academia has an important role. The aim of this project was to design a RSA suitable for political-related local conditions such as: fragmentation and actors’ lack of interest and information.   Methods: To solve uncertainty and to make a solution available, Academia as part of the politics stream, developed a methodological proposal to couple problem and politics streams together into a RSA based on a Scoping Review and ISPOR recommendations.   Results: It was designed a RSA based on extracted Lessons from a Scoping Review and ISPOR good practices. Two specialized hospitals in cancer treatment with well-defined medical protocols were included. Nevertheless, given mentioned political-related local conditions, it was developed a RSA Implantation-Method which considered: design-thinking tools, strategic-momentum elements, negotiation skills, technical expertise and political know-how.   Conclusion: As a complement of the RSA design, it became necessary to develop an Implementation Method.   Academia behaves as a facilitator in the policy formation process and policy window establishment in a fragmented policy-making environment. Given its technical expertise and political know-how, Academia has an enabling role during the RSA deliberative process by articulating policy makers, enhancing evidence-based discussion and reducing uncertainties.
