MONITORING SYSTEM OF PATIENTS WITH CHRONIC NEUROLOGICAL DISORDERS

OBJECTIVES: optimization of pharmacy, specialized on psycho-neurological disorders; integration of medical and pharmaceutical care for chronic patients and their families; developing of clinical protocols and regional formularies. METHODS: information retrieval, comparative analysis, generalization, systematic, target-oriented. oping of clinical protocols and regional formularies. RESULTS: We created complex of epilepsy specialized pharmacy and psychoneurological /outpatient hospital departments. Pharmacy assortment of antyepileptic drugs corresponds with State drug Formulary (7-edition, 2015), unified clinical protocols and adapted clinical guidelines (2014). Pharmacy is systematically improving the algorithm and information support of pharmaceutical care for chronic patients and their families. Consultations, trainings in some aspects of pharmaceutical care for safe and effective drug use are held. Was developed and implemented patient medical passport and special program, which gives an opportunity to diagnose medicinal prescriptions and optimize pharmaceutical care. Range of antiepileptic drugs is 2841 names of medicines considering forms, doses and manufacturers. Pharmacy database includes 187 epileptic patient files: 136 adults and 51 children. After successful implementation for epileptic patients we modify the program for some other chronic neurological disorders, such as: Parkinson's and Alzheimer's diseases, multiple sclerosis.  CONCLUSIONS: Our program allows efficient providing and integration of medical and pharmaceutical care for chronic patients. Developed and implemented patient medical passport allows monitoring of patients health status and efficiency of drug prescriptions. System allows avoiding incompatibility and contraindications in the presence of comorbidities. n. After successful implementation for epileptic patients we modify the program for some other chronic neurological disorders, such as: Parkinson's and Alzheimer's diseases, multiple sclerosis.
