HOSPITAL LENGHT OF STAY AND COSTS IN PATIENTS WITH URINARY TRACK INFECTIONS- ANALYSIS OF US NATIONAL IN-PATIENT DATA FOR 2015 USING ICD-9 AND ICD-10 DIAGNOSES

OBJECTIVES: To examine trends in hospital length of stay and total costs in patients with UTIs. METHODS: The latest available 2015 National Inpatient Sample (NIS) data set of hospital admissions from the Healthcare Cost and Utilization Project was utilized in order to determine the number of hospital admissions for patients with UTI (identified by ICD-9 for Q1-Q3 and ICD-10 codes for Q4). The LOS and costs were compared by payer type, race and age groups. RESULTS: Based on our inclusion criteria we found 505,963 admissions, representing 7.6% of all hospitalizations. The mean age was 69.15 years (SD: 18.77, Median 73 years). The overall mean LOS was 6.92 days (SD: 8.91, Median 5 days). Majority of the hospitalizations were in patients age 60+ (65%), followed by age 45-60 (13%). Mean (SD) LOS by age: 0-18: 8 days (SD: 20, n= 7218), 18-45: 6.25 days (SD: 10.41, n= 49319), 45-59: 7.76 days (SD: 11.02, n= 67718), 60+: 6.83 days (SD: 7.83, n= 381708). The mean LOS by payer: Medicare: 6.74 days (SD: 7.47), Medicaid: 7.88 days (SD: 13.91), Private: 7.01 days (SD: 9.59). The overall mean charges were $59,230 (SD: $102183). The mean costs by age group were 0-18: $84,596 (SD: $265805), 18-45: $54,868 (SD: $115446), 45-59: $70,688 (SD: $127569), 60+: $57,281 (SD: $88621). The mean costs by payer type were: Medicare: $56,196 (SD: $85692), Medicaid: $69,846 (SD: $146748), Private: $65,683 (SD: $125651). CONCLUSIONS: Patients with UTIs incur high costs for public and private payers due to long length of stay in the hospital. Newer more effective treatments are needed to lower the LOS for hospitals.
