VALUE OF INDIVIDUALLY PREPARED DRUGS FOR HEALTH SYSTEM

OBJECTIVES: Individually prepared drugs (IPDs) still play important role in therapeutic practice, especially in individualized therapy, selection of suitable pharmaceutical forms and dosages. IPDs in Slovakia are fully or partially reimbursed by public health insurance in the healthcare system. The value of pharmacists’ work in preparing IPDs is assessed by price per execution(PPE) – price for special pharmacy performances reimbursed by public health insurance. The aim of the analysis is to compare prices of industrially manufactured medicaments (IMMs) with IPDs and to evaluate IPDs availability for patients with respect to the level of PPE. METHODS: Data analysis compared the prices of selected IMMs with their IPDs alternatives. Price of IMMs included payment by public health insurance and patients‘ co-payment under applicable Categorization (April 2015). IPDs prices represented the sum of prices for substances, packaging materials and PPE. RESULTS: Analysis compared seven preparations (Solutio acidi borici 3%, Unguentum acidi salicylici 5%, Pasta cum olei jecoris aselli, Instillatio aluminii acetico-tartarici, Cremor aluminii acetico-tartarici, Solutio acidi salicylici spirituosa 2 %, Suspensio zinci oxydati cum mentholi). IPDs Solutio acidi borici 3% had the same total costs as IMMs, IPDs Unguentum acidi salicylici 5%was even cheaper than the IMMs and the remaining five evaluated products were more expensive. All IPDs were free of co-payments, in IMMs patient always participated in the payment. CONCLUSIONS: Pharmacies often prefer IMMs with patients‘ co-payments despite available free IPDs. One reason may be a fixed price for the IPDs pharmacy performance (PPE), which does not reflect the actual costs incurred for the preparation of a drug. This may also be the reason why IPDs keep disappearing from some pharmacies and patients unnecessarily pay for their health care.
