THE EFFECT OF THERAPEUTIC DRUG MONITORING IN PATIENTS COMPLIANCE- AN IMPROVEMENT IN DRUG EFFICACY AND CORRESPONDING REDUCTION IN TOXICITY USING AMINOGLYCOSIDES

OBJECTIVES: Evaluation of the therapeutic monitoring of Aminoglycosides towards an improved drug efficacy and corresponding reduction in toxicity to ascertain patients compliance. METHODS: Ten patients of different ages ranging from eighteen to twenty eight years in a university health centre were accessed using Gentamycin(Aminoglycosides) injectables that is given intravenously in the treatment of urinary tract infection. Their drug serum concentration were measured taking consideration of therapy, response, potential hazard due to other medications, toxicity and patience compliance. Gentamycin was dosed at 3mg/kg q 6 to 12 hour for the different patients based on the body weight. After obtaining the age and weight of the patients, blood samples were collected at steady state to obtain clinical useful serum concentrations comprising the following information 1. Time of blood collection 2. Dosing time 3. Dose, duration, dosage form 4. Patients demographics 5. Co-medications 6. Indications for monitoring 7. Pharmacokinetic and therapeutic dose of the drug for the differently aged patience were accessed at steady state. RESULTS: Considerable evidence for variability between normal individuals to diseased individuals showed that at steady state (4-5 half life after starting therapy), the plasma concentration was proportional to receptor concentration. The concentration was gotten to be 10microgram/ml for the peak concentration and 1microgram/ml for the trough concentration after the patients has stabilised. The peak concentration occurred 45 minutes after the dosing while the trough concentration was gotten before the next dose. The peak and the trough concentration gave a much improved efficacy and lesser toxicity. The Indications for toxicity gave outcomes for higher therapeutic to toxic ratio at a steady state resulting to greater compliance of the patience with other disease complications under co-medications. CONCLUSIONS: Therapeutic drug monitoring promotes patients management and improves clinical outcomes towards the achievement of patients compliance
