OBJECTIVES: The diabetes register DIAREG allows a coverage of the health services reality of patients with type 2 diabetes (T2D) in Germany through a focus on Patient Reported Outcomes (PRO) and Quality of Life (QoL). The aim of the register is to evaluate clinical outcomes relating to PRO and QoL over the course of the disease. METHODS: Retrospective patient data from the IMS Disease Analyzer constitute the backbone of the register and are augmented through DIAREG by prospective data from physician questionnaires as well as six validated PRO items: Short-Form 36 Questionnaire (SF-36), Audit of Diabetes-Dependent Quality of Life (ADDQoL), Diabetes Treatment Satisfaction Questionnaire (DTSQ), Impact of Weight Quality of Life (IWQoL-Lite), Center of Epidemiological Studies Depression Scale (CES-D), WHO Five Well Being Index (WHO-5). RESULTS: Until March 2016, in total 2,101 patients were registered. 270 patients with completed PRO questionnaires could be evaluated in a cross-sectional analysis and showed the following main results: Patients with a shorter diabetes duration under two years display a trend to an increased depression score in the CES-D (mean CES-D score 20 (SD=9.3) vs. 13 (SD=9.1) in patients with diabetes duration 2–4 years, p<0.001). Depressive patients (based on CES-D score: 0-15 not depressed, ≥16 depressed) showed a significantly decreased QoL (mean ADDQoL score: -1.8 (SD=1.9) vs. -0.9 (SD=1.4); p<0.0001). Insulin-treated patients have significant lower QoL than those treated with oral antidiabetic drugs (mean ADDQoL score -1.7 (SD=1.8) vs. -1.2 (SD=1.6), p=0.0007). Patients suffering hypoglycemic events reported a significantly decreased QoL (mean ADDQoL score -2.9 (SD=3.3) vs. -1.1 (SD=1.5), p=0.0032). CONCLUSIONS: DIAREG delivered first cross-sectional insights with practical implications for the treatment of diabetes patients. In the future DIAREG will allow answering longitudinal research questions regarding QoL in T2D. DIAREG thus makes a valuable contribution to health services research in T2D.
