BURDEN OF VISUAL IMPAIRMENT DUE TO MYOPIC CHOROIDAL NEOVASCULARISATION (MCNV) IN TURKEY

OBJECTIVES: The study aims to determine the annual direct costs of mCNV in Turkey from the payer [Social Security Institution (SSI)] perspective. METHODS: Delphi technique was applied to determine the type and the amount of resources used including epidemiology, diagnosis, treatment of symptoms and side effects, follow-up visits, hospital admissions based on physicians clinical assessment by gathering experts in a meeting to build up consensus. Unit costs of resources used in outpatient and inpatient were obtained from official lists of SSI and total costs were calculated using the opinions of physicians on resource utilisation. Calculations were made in TRY and converted to EUR using the current currency (May 2016). RESULTS: In mCNV, intravitreal injection (IVI) with ranibizumab is used in approximately 99% of patients, with the remaining 1% receiving verteporfin photodynamic treatment (vPDT). IVI patients are admitted to the ophthalmology outpatient clinic five times during the year, whereas vPDT patients are admitted four times during the year. Ranibizumab costs €376.07, whereas IVI intervention cost is €21.16 for two administrations. Verteporfin's unit cost is €428.2 and PDT intervention cost is found as €102.63. Ophthalmology outpatient admission fee is calculated as €11.57. Cataract and retinal detachment costs were added to the total cost as complications and side effects. Mean annual follow up cost was found as €70.94. Mean service and intervention cost was calculated as €778.45. The mean cost of side effects is €1.83 on annual basis and per patient. CONCLUSIONS: In light of these components, annual mean mCNV cost per patient is found as €915.07. The number of mCNV patients on treatment are estimated as 250 and the total annual cost of mCNV for Turkey is €228,767.50. This amount is 0.0006% of expected total health expenditure of Turkey in 2016.
