Work Productivity and Activity Impairment in Patients with Severe Asthma Treated with Mepolizumab in a Real-World Setting

Objectives: Severe asthma is associated with substantial productivity loss and activity impairment. In the real-world REALITI-A study, mepolizumab reduced clinically significant exacerbations and oral corticosteroid use in patients with severe asthma. Although mepolizumab improved activity impairment and work productivity in clinical trials, real-world data are limited. This post hoc analysis assessed the real-world impact of mepolizumab treatment on work productivity and daily activity impairment.

Methods: REALITI-A is a 2-year international, prospective, observational study, enrolling adults with asthma following a physician decision to initiate mepolizumab treatment (100 mg subcutaneous). We assessed working status, productivity, and daily activity impairment, using the Work Productivity Activity Impairment (WPAI) Asthma questionnaire, completed at baseline and optionally every 3 months, at routine healthcare visits. We report interim results for the full study population at 1 year.

Results: Overall, 822 patients were treated with mepolizumab. Versus baseline, mepolizumab treatment was associated with least squares mean reduction in the percentage of absenteeism (11.1% [N=337] vs 2.1% [N=102]), impairment while working (36.4% [N=316] vs 13.6% [N=100]), overall work impairment (38.5% [N=316] vs 15.1% [N=100]), and daily activity impairment (52.1% [N=768] vs 29.7% [N=214]). A small increase in the proportion of patients who were employed at 12 months (51.4%; n=112/218) was observed versus baseline (46.9%; n=368/784). Of the 208 patients with employment status information available at baseline and Month 12, most patients had no change from baseline in their employment status (48.1% remained employed, 43.8% remained unemployed); 3.4% and 4.8% changed to employed and unemployed, respectively; similar results were observed when employment status was analyzed using the latest post-baseline questionnaire data (n=560).

Conclusions: Although data before and following mepolizumab treatment were incomplete, these findings suggest that, in addition to the real-world clinical benefits previously reported, mepolizumab treatment improves work productivity and reduces daily activity impairment.
