HEALTH TECHNOLOGY ASSESSMENT IN PLURALISTIC HEALTH CARE SYSTEMS: CHALLENGES AND PRELIMINARY LESSONS LEARNED

PURPOSE:

The purpose of the workshop is to explore the challenges of conducting and using HTA in pluralistic health care systems with multiple payers. Potential solutions will be discussed, using examples from a range of health care settings.

DESCRIPTION:

Kai Yeung (12 mins) will discuss the preliminary findings of a literature review conducted for the ISPOR HTA Council Working Group on Challenges of HTA in Pluralistic Healthcare Systems. Potential challenges include; developing a critical mass (budget and expertise) for conducting national or local HTA; contextualizing study findings to suit individual payers’ needs; sharing experiences on the use of HTA for decision-making in a competitive environment, where some information may be proprietary; incorporating HTA evidence into insurance products when this may not be easily understood by health plan purchasers. Emily Tsaio (12 mins) will share experiences from the perspective of a US health plan. In particular, what local resources are required, what can be gleaned from national assessments (eg undertaken by organizations such as the Institute for Clinical and Economic Review) and what additional analyses need to be conducted locally? Federico Augustovski (12 mins) will discuss the challenges in a Latin American setting, where government health services operate alongside those funded and provided by social insurance or the private sector. In particular, can government funded HTA meet the needs of a range of payers with different levels of budget and different decision rules. He will also discuss some approaches for determining the opportunity costs of health, as well as setting budget impact and affordability thresholds in pluralistic health care systems. Michael Drummond (24 mins) will moderate the workshop and orchestrate the discussion. In addition to responding to several polling questions, workshop participants will be encouraged to share their experiences of HTA in pluralistic systems, from a payer, manufacturer or researcher perspective.
