THE TELEDERMATOLOGY EXPERIENCE IN BRAZIL

OBJECTIVES: Present the results of the teledermatology service performed in a Brazilian state from 2016 to 2018. METHODS: A survey was conducted in a federal government database of teledermotology services from January 2016 to December 2018. The classification of the exams was given in categories according to Manchester Triage system. RESULTS: During the period considered, 79,411 tests were performed at the teledermatology. In 2016 and 2017, respectively, 23,165 and 22,929 examinations were performed. In 2018, 33,317 (42.0%) exams were registered. Most of the exams were women (63.7%), aged between 0 and 105 years, with a mean of 50.4 (± 20) years. Between 2016 and 2018, 40.2% of the exams were classified as white or blue risk category, 40.0% as green and 19.8% as yellow. There was no significant difference in the distribution of categories between the observed years. The main diagnoses among the white classification exams were seborrheic keratosis (32.9%), chronic photodamage (28.0%) and melanocytic nevus (19.6%). The exams that received blue classification were identified as dermatoses of different types, the most frequent being eczematous (35.4%) and infectious (30.8%). Of the 15,693 tests classified as yellow risk category - which led to tertiary referrals - 75.2% were suspected of non-melanoma skin cancer, 8.6% were moderate or severe psoriasis and 6.6% were melanoma. CONCLUSIONS: Teledermatology care can result in many cases being resolved without the need for referral, and thus prevent complications and delay patient care. In addition, the teledermatology services used may be a reliable and cost-effective alternative to accessing dermatological care. In addition, there is an innovation in the organization of health actions and services in Brazil with the implementation of teledermatology.
