DIRECT COST OF A MATERNAL CHILD CARE PROGRAM, AND THE IMPACT IN THE INTENSIVE CARE UNIT HOSPITALIZATIONS OF PREGNANCY WOMEN AND NEWBORN INFANTS, IN A HEALTH INSURER IN COLOMBIA.

OBJECTIVES Estimate the direct cost of a Maternal Child Care Program (PAMI) created in 2017, which monitor pregnancy and newborn infants from the 20th week of pregnancy to the mediated puerperium; in addition to assessing the impact it has had on the hospitalization of pregnancy women and newborn infants, in the intensive care unit (ICU). METHODOLOGY A retrospective study of 8658 pregnancy women affiliated with a health insurer in Cartagena-Colombia who received vaginal delivey or caesarean section during the period between January 2017 and November 2019 was conducted; 7349 were treated within the PAMI and 1309 served outside the PAMI. The frequency of pregnant women and newborn infants with hospitalizations in the ICU was determined; the average annual costs were estimated. RESULTS
- The hospitalization rate in neonatal ICU within the PAMI, ranged monthly between 2% and 9% with an average of 6 and a standard deviation (SD) of +/- 1.5.
- The rate of hospitalizations in neonatal ICU outside the PAMI, ranged monthly between 0 and 50% with an average of 13 and a SD of +/- 11.
- The pregnant women hospitalization rate, in the ICU inside and outside the PAMI, ranged between 0.3% and 1.2% monthly.
- The average annual cost of vaginal deliveries within the PAMI for 2017, 2018 and 2019 adjusted to the value of inflation to 2019 year, was $394 USD, $604 USD, $695 USD respectively.
- Adherence to the Maternal Child Care Program has been satisfactory; were attended within this, 85% of the pregnant women in Cartagena branch office during the established period.
- The average hospitalization rate in neonatal ICU was 2.1 times higher outside the PAMI.
- The hospitalization rate in maternal ICU dropped globally inside and outside the PAMI, during the period of time studied.
