OBJECTIVES: A model of implementation in VBHC in Portugal is important for the development of a needed model in healthcare. An Integrated Practice Unit (IPU) was constructed providing the full care cycle for severe asthma patients. The use of both provider and patient reported outcome data was described along the value chain for severe asthma. A Standard Set for Severe Asthma Outcomes together with case-mix variables and timelines was agreed upon within the IPU. A secure prototype of the implementation procedure was designed to measure PROs during the full cycle of care phase. To deliver it was needed to.

METHODS: The project was designed to have a basic model of education on VBHC concepts, along with the sample of pharmaceutical professionals and it aims to contribute to the implementation of a VBHC system at severe asthma approach in patients followed several hospitals in Portugal. The project was developed in a time frame of 6 months in 8 workshops with 22 stakeholders along with the national coverage of public and private hospitals.

RESULTS: Dedicated resources, change of culture and practice, and improved knowledge and awareness about (VBHC) is essential. Our proposed framework aims to serve as a blueprint for implementation of VBHC in daily care. Along the time of research in 6 months of workshops and evolution of the process and literature in evidence it was able of development a structure that aligns PROMs, Pathways, Clinical Indicators in a Multidisciplinary provider and patient perspectives were obtained via focus groups and literature review.

CONCLUSIONS: A new approach based on clinical evidence, through the accountability of all stakeholders involved in the results obtained in the care cycle, organized on the basis of patient preference responding to the growing needs of society and the health of the population that translates the adoption of best practices in health.
