Loss of Functionality and Low Scores on Quality-of-Life Scales in Obese and Non-Obese Sarcopenic Patients with Head and Neck Cancer: Cross-Sectional Study Based on Real Data World.

The findings regarding a possible relationship between sarcopenia and quality of life (QoL) in cancer patients are conflicting. In patients with head and neck cancer (H&NC), there have been very few reports; moreover, little is known about the association of sarcopenic obesity with the QoL of cancer patients.

OBJECTIVES: To explore the impacts of sarcopenia and sarcopenic obesity on the scores on QoL scales and several indicators of nutritional status in patients with H&NC.

METHODS: In this cross-sectional study, the relationships of the scores on the EORTC QLQ C-30 and EORTC QLQ-H&H35 instruments with body composition, hand-grip strength, gait speed, total lymphocyte count, and the levels of albumin, cholesterol, and C-reactive protein were investigated in 71 H&NC patients (aged between 40 and 80 years).

RESULTS: The prevalences of sarcopenia and sarcopenic obesity were 48% and 28%, respectively. The nonsarcopenic (SCP) patients had greater hand grip strength, higher scores for physical functioning scale [QLQ C-30: NSP group 93 (83 – 100); S group 73 (52 – 88); SO group 83 (53 – 93), p 0.009] and fatigue [QLQ C-30: NSP group 11 (0 – 22); S group 39 (30 – 67); SO group 44 (14 – 56), p 0.004] and lower levels of C-reactive protein than the sarcopenic (S) and sarcopenic obesity (SO) groups. No differences were observed between these latter two groups.

CONCLUSIONS: In this study, the physical functioning scale score was better in H&NC patients without sarcopenia than in patients with sarcopenia or sarcopenic obesity. Due to the possibility that obesity can mask low skeletal muscle reserves, it is important to perform a detailed assessment of nutritional status to detect sarcopenia early, as it leads to the loss of functionality
