Cost Analysis of Using Aspiration Systems for the Treatment of ACUTE Ischemic Stroke in Turkey

OBJECTIVES: Cerebrovascular diseases and stroke is the third leading cause of death in the world and include spectrum of diseases in which one area of the brain is affected temporarily or permanently due to ischemia or hemorrhage. Every year 17M people in the world suffer from stroke and 6M people are lost due to this reason. Hemorrhagic stroke is caused by bleeding as a result of rupture of brain vessels. Ischemic stroke is a result of the clot formed in the brain vessel where blood flow to the brain decreases or stops and 80% of the cases are in this group. The objective of this study is to evaluate the clinical advantages and cost consequences of using aspiration catheter in ischemic stroke cases.

METHODS: A decision-making model was developed for cost analysis from the perspective of the Social Security Institution. Budget impact analysis was performed by comparing 1 stent retriever and 1 aspiration catheter instead of 2 stents retriever.

React aspiration catheters are indicated for placement of interventional/diagnostic devices into the peripheral vasculature and neurovasculature. The catheter and Riptide™️ Aspiration System are used in the revascularization of patients with acute ischemic stroke secondary to intracranial large vessel occlusive disease (within the internal carotid, middle cerebral M1-2 segments, basilar and vertebral arteries) within 8 hours of symptom onset.

RESULTS: Analysis provide 588$ budget saving per case with the perspective of Turkey’s Social Security Institution (SGK). Estimated public budget savings will be 2M$ in three years. The model and real word evidence shows decrease in dependency ratio to healthcare facilities, reduction in survivor functional dependence and increase patient quality of life.

CONCLUSIONS: Model results propose that aspiration catheter is a more econonomical treatment option than 2 stent retrievers implementation, which also results in better health outcomes for complete reperfusion and functional dependence.
