Budget IMPACT MODEL of Subcutaneous Immunoglobulins in the Treatment of Chronic Inflammatory Demyelinating Polyneuropathy (CIDP) in France

OBJECTIVES : To develop a budget impact model utilizing the French Social Security perspective, to assess the economic implications of switching patients with CIDP from intravenous immunoglobulin (IVIg) maintenance therapy in the hospital setting to subcutaneous immunoglobulin (SCIg) maintenance therapy in the homecare setting.

METHODS : The model was developed in Excel and includes CIDP epidemiology data validated by the French HTA agency (HAS), projections in terms of patients switching to SCIg on a time horizon of 5 years, IVIg and SCIg dosage and frequency of administration from SmPC, hospitalization costs for IVIg administration based on French PMSI database and homecare provider costs. Sensitivity analysis was conducted to estimate parametric uncertainty around model outcomes.

RESULTS : 10% decrease of the total annual cost per patient was observed from switching IVIg patient receiving treatment in hospital to SCIg treatment delivered at home. With an annual cost estimated at €70,203 per IVIg patient and €63,346 per SCIg patient, in the base case scenario, savings over 5 years following SCIg launch in CIDP should reach 8,1 M€. Sensitivity analysis showed that cost of hospitalization for inpatient and annual number of IVIg injection had the strongest magnitude of impact on model results. Within a range of € 56,000 to € 17,4 M, savings are systematically produced by a switch to SCIg.

CONCLUSIONS : The model showed that this first SCIg approved in CIDP is a cost saving treatment from a social security perspective reducing hospital visits for patients.
