Influenza Vaccination and Its IMPACT on the Availability Intensive Care Unit BEDS during the COVID-19 Pandemic in Colombia

OBJECTIVES: A major concern for healthcare systems during coronavirus disease 2019 (COVID-19) pandemic is the scarcity of Intensive Care Unit (ICU) beds to attend critically ill patients. Reduced mobility of citizens in Colombia, represents a risk of reduction in influenza vaccination. Vaccination is known to reduce hospitalization of patients for influenza-associated disease. The objective is to calculate the potential ICU beds non-available to attend COVID-19 given the potential lower influenza vaccine coverage, and its consequent costs, from a healthcare payer perspective. METHODS: A literature and public databases review was performed to identify ICU beds capacity, costs, and occupancy related to influenza and COVID-19. With the expected cases and resources use, costs for ICU beds with TIV or QIV influenza vaccination versus no influenza vaccination, were calculated and compared. RESULTS: Colombia had 5610 adult ICU beds, with 818 used for confirmed COVID-19 cases by the end of Q2. The expected occupancy for influenza in the same period corresponds to 4139 without vaccination, 2831 with TIV, and 2548 with QIV vaccination. Costs associated are USD$5,07 million without vaccination, USD$3,47mm with TIV and USD$3,12mm with QIV vaccination. ICU beds required from not vaccinating represent USD$1,60mm, with an expected cost of USD $1.226 per case. Vaccination with QIV could save an additional USD$346.942. CONCLUSIONS: The expected occupancy of adult ICU beds with COVID-19 cases and influenza peak for Q2 is around 65% with TIV vaccination. With the projected increase in coronavirus cases, this occupancy is expected to increase close to full capacity. Influenza vaccination can increase ICU beds availability for COVID-19. Influenza competes for resources required during the pandemic with costs that could be better used to attend and increase ICU capacity. Effectiveness from a QIV vaccine can further reduce the need for ICU beds. Influenza vaccination is advisable to avoid the effects of preventable cases.
