Morbidity, Mortality and Risk Management Indicators in Hemophilia: Third Payer in Colombia

OBJECTIVES: The health systems in the world suppose changes for outcomes improvement in population health. The management of patients with high-cost pathologies such as hemophilia has allowed the establishment of indicators to measure clinical and management outcomes throughout the world. Thus, the objective is to analyze the relationship of risk management indicators of patients with hemophilia and morbidity and mortality rates by insurers in Colombia in 2019. METHODS: Based on the national hemophilia registry in Colombia, a secondary analysis cross-sectional study was proposed. It was estimated the number of affiliates, prevalence, and mortality of hemophilia by each insurer and the risk management indicators of each entity for 2019. RESULTS: The morbidity and mortality and risk management indicators were obtained for 45 insurers. The prevalence and mortality of hemophilia was 5 per 100,000 affiliates and 3.5 per 1,000,000 affiliates in the country, respectively. The proportion of hemophiliac patients in prophylaxis with chronic hemophilic arthropathy is directly related to the number of affiliates, prevalence, and mortality (p <0.05). The proportion of patients evaluated by a multidisciplinary team has an inversely proportional relationship with prevalence and mortality (p <0.05). The bleeding rate hospitalizations have a directly proportional relationship with mortality and an inversely proportional relationship with the prevalence and number of affiliates (p <0.05). CONCLUSIONS: The follow-up of patients of a high-cost pathology such as hemophilia is a challenge in the clinical management of insurers. The country has a relatively low prevalence and mortality and is a pioneer in the region in the development of indicators that assess the clinical management of these patients. The continuous and close management of hemophiliacs through multidisciplinary teams and the management of complications principles affect the quality of life of patients and their families.
