EVALUATION OF PATIENTS DEPRESCRIBED OF H2 RECEPTOR ANTAGONISTS AND PROTON PUMP INHIBITORS USING KWAS QUESTIONNAIRE

OBJECTIVES: To determine the Knowledge, Willingness, Attitude and Shared decision making (KWAS) of patients employing a questionnaire.

METHODS: Prospective interventional study was conducted for duration of 7 months from October 2018 to April 2019 in the Department of Gastroenterology, M.S. Ramaiah Hospital, Bangalore, India. Patients aged >18 years, consuming H2RAs and PPIs > 4 weeks were included. The KWAS questionnaire was prepared and validated for face and content validity, reliability by experts in the Gastroenterology department. The cronbach’s alpha value was found to be 0.874. The KWAS questionnaire was administered at baseline and after 8 weeks of deprescribing. The investigator created awareness and provided counseling on appropriate measures of medication utilization.

RESULTS: Among 367 patients analyzed, significant association was observed among the patient’s KWAS before and after deprescribing. The values obtained at baseline and post counselling at 8th week of follow up showed significant increase with z- value -4.388, p value of 0.000 (<0.05) for knowledge, z- value -11.29, p value of 0.000 (<0.05) and for willingness z- value -9.099 and p value of 0.000 (<0.05) for attitude. It was found that 219(59.67%) and 251(68.39%) of patients were interested to be part of SDM before and after deprescribing about H2RA and PPIs. Significant improvement in patients choosing pharmacist in deprescribing of H2RA and PPIs seemed to increase from 123(33.51%) to 184(50.1%) after counseling. Therefore after effective counselling for deprescribing of H2RA and PPIs implementation of life style modifications increased from 145(39.5%) to 205(55.85%).similarly patients concern towards monitoring and follow up by physician after deprescribing increased from 121(33.24%) to 231(62.94%) after counseling.

CONCLUSIONS: The KWAS response towards chronic redundant use of these medications was poor before and improved after deprescribing. Therefore effective counseling and education to patients would help in reducing the adverse effects and cost constraints.
