TOLVAPTAN REDUCES HEALTHCARE BURDEN IN PATIENTS WITH STAGE 1-3 ADPKD IN SPAIN

OBJECTIVES : This model aims to study the potential economic benefits of treating patients with ADPKD in Spain with tolvaptan, according to new evidence produced by the ADPKD outcomes model. Real world use of tolvaptan doses is also summarized. METHODS : For calculations, the following evidence were taken into account: last publications of the ADPKD outcomes model (a model predicting the impact of a delay of ESRD by tolvaptan), analysis of the market from internal reports within the 12-month-period after launch to compare real word use vs pivotal trial, and public prices of dialysis procedures. RESULTS : According to 2017 public prices in Spain, the cost of a dialysis is €39,571.43 (peritoneal) and €53,186.14 (hemodialysis). Since relative use of these procedures are 6.4% and 91.1% respectively, the mean cost for dialysis in Spain is €52,163.55/year. Recent publications show that in a time horizon, 15 years, every 1.000 patients treated with tolvaptan, there is a potential reduction of 420 life years of dialysis, translating to a reduced healthcare burden on the Spanish Health System of €21.908.691. In TEMPO 3:4 (NCT00428948), the percentage of patients on 60, 90 and 120 mg doses of tolvaptan were 24%, 21%, and 55%, respectively. Analysis of the 12-month-period following the launch of tolvaptan in Spain (in March 2017), found real world use of the respective doses was consistent, at 20%, 21%, and 59%, respectively. CONCLUSIONS : Benefit according the ADPKD outcomes model, which estimates the impact of a delay to ESRD from using tolvaptan, may facilitate a potential savings for the Spanish Health System of €21.908.691. Real world use of varying doses of tolvaptan in Spain confirms those used in pivotal trial clinical trials.
