OBJECTIVES

The Mexican health system offers universal access to antiretroviral (ARV) drugs. The economic burden, despite off-patent antiretroviral, antirretroviral therapy cost remains high. Even with a low prevalence, at the end of 2018, 164,074 Mexicans are living with HIV and require ARV treatment. BIKTARVY® (BICTEGRAVIR/EMTRICITABINE/TENOFOVIR ALAFENAMIDE, B/FTC/TAF) has the potential to low the current economic burden when compare to the combination raltegravir (RAL) or dolutegravir (DTG) plus backbone (Abacavir/lamivudina, ABC/3TC, Emtricitabina/tenofovir disoproxil fumarato FTC/TDF and succiante FTC/TDS), current therapies use in the treatment of adults with naïve human immunodeficiency virus type 1 (HIV-1) in Mexico.

METHODS

The aim of this Budget Impact Analysis is to assess the difference in the current annual expenditures versus a new scenario, when B/FTC/TAF is included and increases its public market share (30% to 70%) form the starting scenario. Current Public Mexican market shares for triple regimens containing first-choice (RAL or DGT) plus alternative backbones (ABC/3TC, FTC/TDF and FTC/TDS), in the new scenario those combintations are compared to B/FTC/TAF. The per year cost for each regimen only include drugs acquisition.

RESULTS

According to current prescribing trends, in the next 5 years about 423 patients starting HIV therapy will receive ABC/3TC, FTC/TDF. In the new scenario, when 30% share of patients are elegible the savigns generate by B/FTC/TAF are $693,325 USD ($567,423-$801,002), when B/FTC/TAF share increases to 50% the cumulative saving are $912,767 USD ($824,112-$1,002,209), finally when B/FTC/TAF share is 70% the savings are $1,289,563 USD ($965,323-$1,724,998).

CONCLUSIONS

If first-choice (RAL or DGT) plus alternative backbones ABC/3TC, FTC/TDF and FTC/TDS are displaced, by including B/FTC/TAF (BIKTARVY®) in the mexican formulary, large and important savigs are to be expected.
