COST-EFFECTIVENESS ANALYSIS OF USING IODINE IMPREGNATED DRAPE TO PREVENT SURGICAL SITE INFECTION IN CHINESE SURGICAL PATIENTS

OBJECTIVES : Surgical site infection is a common nosocomial infection and surgical complication in postoperative patients and bring serious disease and economic burden. The purpose of this study was to evaluate the cost-effectiveness of using Ioban 2 drape to prevent SSI in Chinese surgical patients. METHODS : This study systematically reviewed the existing literatures for SSI incidence rates and costs of SSI in different types of surgeries. According to the results of the systematic review, a decision tree model was established, and a 1000-patiens cohort simulation was conducted from the perspective of healthcare system of China. ICER was calculated in base-case analysis. The robustness of the base-case analysis results was tested by one-way sensitivity analysis and probabilistic sensitivity analysis. RESULTS : Compared with non-drape, Ioban 2 drape can save 50,508 yuan/1000 patients and avoid SSI 11.94 cases /1000 patients in cardiovascular surgery. In orthopedic surgery, it can save 1,834,507 yuan/1000 patients and avoid SSI 116.67 cases /1000 patients. Compared with standard drape, Ioban 2 drape can save 404,503 yuan/1000 patients and reduce SSI 27.17 cases /1000 patients in orthopedic implant surgery. In neurosurgical surgery, Ioban 2 drape can save 941,219 yuan/1000 patients and reduce SSI 35.16 cases /1000 patients. In gynecologic surgery, it can save 162,621 yuan/1000 patients and reduce SSI 100.00 cases /1000 patients. The results of one-way sensitivity analysis were consistent with the base-case analysis. The CEACs showed that the probabilities of Ioban 2 drape being cost-effective was much higher than standard drape/non-drape in all types of surgeries when the willingness-to-pay threshold increased from 0 to 10,000 yuan. CONCLUSIONS : Compared with non-drape, in cardiovascular and orthopedic surgery, and compared with standard drape, in orthopedic implant, neurosurgery and gynecologic surgery, Ioban 2 drape can reduce the total cost and the number of SSI, Ioban 2 drape is the absolute dominant strategy.
