MODEL Based Pharmacoeconomic Evaluation of Medication for Depression Disorder: A Systematic Review

OBJECTIVES

:
To provide an overview of published cost-effectiveness models in depressive disorder with a focus on the methods employed, discuss the suitability of the models for simulating the course of depression.

METHODS

:
A literature search was conducted in PubMed, The Cochrane Library, EMbase, CNKI, Wanfang, and VIP databases between 1 January 2013 and 7 January 2020. The search strategy was developed using a combination of Medical Subject Heading (MeSH) and free-text terms. CHEERS checklist was applied to measure the quality of reporting.

RESULTS

:
A total of 20 articles from multiple countries were included, of which 17 were of high quality. Eight studies applied decision tree (DT) models, five studies applied Markov models, while six studies combined both DT and Markov models. Discrete event simulation (DES) model was developed in one study. Except for the lifetime simulation of DES model, time horizons of the other models were up to two years. The perspectives were payer (13/20), society (2/20) or both (4/20). Clinical and utility parameters were mostly obtained from existing literatures, clinical trials. Three studies adjusted the utility value by using the utility score system of local people. Cost parameters were mainly from the public data. Outcomes were mostly QALYs (15/20). A few studies verified the surface validity of the models (8/20).

CONCLUSIONS

:
Although DT model is the most used, combination of DT and Markov model could reflect the stage-specific treatments during disease progression. Individual level Markov and DES models may be more practical for the course of depression by including patient characteristics and heterogeneity. However, direct comparisons between the available modelling techniques are necessary to yield firm conclusions. In addition, it is necessary to verify the validity of the model as well as optimizing the quality and applicability of data sources.
