The Health-Related Quality of Life in Different Disease Population Based on EQ-5D-5L: A Systematic Review

OBJECTIVES : EQ-5D-5L is a generic utility-based instrument introduced by Euroqol Group to measure health-related quality of life (HRQoL) in 2005. Since then it has been increasingly applied in various disease population with good reliability and sensitivity. This study aimed to summarize health states utility value (HSUV) elicited using EQ-5D-5L for patients with different diseases in cross-sectional studies. METHODS : MEDLINE, EMBASE, Web of Science, and the Cochrane Library were searched from 1st January 2012 through 31st October 2019. Eligible cross-sectional studies reported HSUVs measured by EQ-5D-5L for any specific disease population. Language was limited in English. Additional hand-searching reference lists of retrieved studies was performed to identify more researches that met inclusion criteria. Methodology quality was assessed by Agency for Health Research and Quality (AHRQ) checklist. RESULTS : In total, 9400 records were identified and 99 studies met the inclusion criteria. For these included studies, 50 kinds of diseases with 102,232 patients were analyzed. The health utility for diabetes mellitus with different severity and comorbidity ranged from 0.31 to 0.99; neoplasms from 0.62 to 0.90; multiple sclerosis from 0.31 to 0.78; cardiovascular disease from 0.56 to 0.88; chronic obstructive pulmonary disease from 0.68 to 0.79; HIV infection from 0.65 to 0.90; chronic kidney disease from 0,37 to 0.89; hypertension from 0.80 to 0.88; Prader-Willi syndrome from 0.40 to 0.81; ulcerative colitis from 0.66 to 0.86 and psoriasis from 0.74 to 0.90. However, half of diseases only reported a single HSUV in the study. To calculate HSUV, country-specific EQ-5D-5L value set was needed. The value set of England was mostly used. When it was not available, crosswalk methods could be applied. Of the 99 included studies, 45 reported using crosswalk methods. CONCLUSIONS : EQ-5D-5L has been one of the mostly used preference-based measures to assess HRQoL in patients worldwide. Country-specific value set should be applied when it’s available.
