Motivation for Incorporation & Access for Tetravalent Immunization Against Dengue. A Case Study of Vaccine Innovation

Problem Statement: Dengue is the most important reemerging mosquito-borne viral disease worldwide. The incidence of dengue in Brazil has been frequently high, and the number of cases in the country has at some point in time represented up to 60% of the dengue reported cases worldwide. Our problem addresses vector distribution, dengue outbreaks, circulating serotypes and genotypes, and prevention approaches being utilized in Brazil. The burden of dengue in Brazil is increasing and likely underestimated.

Description: A RWD study of prevalence and direct costs related to dengue management in Brazil's private healthcare from 2015 to 2020 , showed that the median cost per hospitalization in 2015 was US$ 486.17, and in 2020, it reached US$ 696.72. The median cost of a case seen at an emergency room ranged from US$ 97.78 in 2015 to US$ 118.16 in 2017. The National Health Surveillance Agency (ANVISA) approved it and is based on the results of robust study with clinical data follow-up by four and a half years and to date show sustained efficacy and no major safety risks . It is the only Dengue vaccine approved in Brazil for use in individuals without the need for pre-vaccination testing .

Lessons Learned: Several vaccines against dengue have been and are still under development , however , in Brazil, only two vaccines against this virus have been licensed . It is the first immunizer authorized in Brazil for people who have never come into contact with the dengue virus, but it can also be applied to those who have already had the disease. In clinical trials, it was shown to have an overall efficacy of 80.2% against dengue caused by any serotype after 12 months of the second dose.
