EPIDEMIOLOGY, TREATMENT, AND HEALTH RESOURCE USE OF GOUT PATIENTS IN GERMANY- RESULTS FROM ANALYSIS OF A CLAIMS DATABASE

OBJECTIVES: Gout is the most common inflammatory arthritis in Germany. This study aimed to provide an up-to-date analysis on the prevalence and incidence of gout, associated comorbidities, current treatment patterns, as well as health ressource use (HRU) and costs METHODS: The InGef database of ~4 million healthcare claims, was used. Patients were included based on hospital discharge/ambulatory diagnoses of gout (ICD10 M10)/ hyperuricemia (E79.0) in combination with uric acid tests, prescription of urate lowering therapy (ULT), and treatment of gout flare. Prevalent cases were analyzed for comorbidities, treatments, HRU, and costs. Data of gout patients were compared to an age- and sex-matched control group without gout RESULTS: Prevalence was 1.63% corresponding to 1.3 million patients, incidence was 0.45%. Approximately 46.7% of patients were in the working age group of <65 years. Gout patients had more comorbidities, hospitalizations, physician’ and specialist’ visits, and medications than patients without gout (p<0.001). Mean hospital costs were higher (gout patients: 2,498€ vs. non-gout patients: 1,667€), as well as drug treatment costs (1,148€ vs. 815€), ambulatory care costs (971€ vs. 722€), and costs for remedies and aids (427€ vs. 332€) (p<0.001). Consequently, overall mean costs were significantly higher in gout patients (5,193€) as compared to patients without gout (3,642€) (p<0.001). Also gout patients took ~20% more sick leave than non-gout patients (6.93 vs. 4.63 days) CONCLUSIONS: Gout is highly prevalent and associated with a higher number of comorbidities, physician visits and medications, with substantially increased health resource use and overall costs, as well as a higher number of sick leave days, compared to patients without gout
