EFFECTIVENESS OF THE HEPATITIS C VIRUS, NS5A L31/Y93 GENE MUTATION TEST- SYSTEMATIC REVIEWS

OBJECTIVES: The purpose of this study was to evaluate the effectiveness of hepatitis C virus, NS5A L/31Y93 gene mutation test for predicting the sustained virologic response (SVR) upon planning the Daclatasvir and the Asunaprevir treatments by screening the mutation on NS5A-L31 and NS5A-Y93 of the gene sequence of patients suffering from a chronic liver disorder caused by Hepatitis C virus of the genotype 1b. METHODS: The literature search was conducted using eight Korean databases and foreign databases including Ovid-MEDLINE, EMBASE, and Cochrane Library from 13 October 2016 to 17 November 2016. Of the 468 articles that were initially identified, 11 studies were included in the final assessment. Two reviewers independently assessed the quality of the included studies and extracted data. The quality of the studies was assessed according to the Scottish Intercollegiate Guidelines Network (SIGN) tool. RESULTS: The effectiveness of the hepatitis C virus, NS5A L31/Y93 gene, mutation test was assessed through the effects on the medical outcome (prognosis, change of therapeutic agent) and the mutation detection rate. For prognosis, 10 articles reported on 12-week or 24-week SVR. In 3 articles, the odds ratio of the SVR was reported as 2.10~19.64 in the group without any mutation. In 7 articles, the ratio of the patients who showed SVR according to the mutation on the NS5A gene was reported as 90.3%~100.0% in the group without any mutation and 40.0%~64.3% in the group carrying a mutation, indicating a high level of SVR in the group without any mutation. CONCLUSIONS: It is an effective test for screening the mutation on NS5A-L31 and NS5A-Y93 of the gene sequence of patients suffering from a chronic liver disorder caused by Hepatitis C virus of the genotype 1b, so that the SVR can be predicted for Daclatasvir and Asunaprevir treatments.
