COST ANALYSIS OF AN ERROR TARGETED INTERVENTION TO REDUCE OR ERADICATE INHALER ERRORS IN ASTHMA PATIENTS IN THE UK

OBJECTIVES: CRITIKAL (Price et al METHODS: A cost analysis of asthma inhaler errors was developed, based on a cost-utility analysis by Forster et al (2018), to estimate the costs of inhaler errors and the potential savings that could be achieved by introducing an error targeted intervention. The number of patients with asthma aged 16 years and over receiving fixed dose inhaled corticosteroids and long acting beta agonists was estimated. The cost of each inhaler error was estimated based on the number of patients making the inhaler error, experiencing uncontrolled asthma and exacerbations over five years.

RESULTS: The highest costs were estimated for the insufficient inspiratory effort error with a dry powder inhaler. This was due to its higher frequency of handling error and association with uncontrolled asthma and exacerbations. Reducing this error by 25% through the introduction of an error targeted intervention could lead to cost offsets to the NHS of £34,479,669 over one year, due to fewer patients experiencing uncontrolled asthma and severe asthma exacerbations (6,709 and 15,410, respectively). Eradicating the error fully could lead to cost offsets to the NHS of £137,918,677 over one year.

CONCLUSIONS: Our analysis showed that error targeted interventions such as training programmes or the introduction of new inhaler devices that potentially reduce or eradicate specific inhaler errors can lead to better asthma outcomes, cost-offsets and a reduction in resource utilisation.
