MANAGEMENT AND OUTCOMES OF ACUTE KIDNEY INJURY

OBJECTIVES: To study  the Management and Outcomes of Acute Kidney Injury. METHODS: Retrospective  observational  study,  carried  out  in  a  South  Indian  Tertiary  Care Teaching Hospital. Institutional Ethics Committee approval was obtained prior to the study. As per the study criteria, Patients of age ≥18 year admitted during July-December 2012 under the ICD 10 coding for AKI (N17.9) were included for the study. Patients who had undergone renal transplantation and chronic haemodialysis were excluded. Pre-designed Case Report Form (CRF) was used for data collection. Outcomes were analyzed using the descriptive statistics and  categorical variables were compared using the X test using SPSS v20.0. RESULTS: During the six months study period,297 AKI patients were admitted. 211(71%) received 0.9% Normal Saline along with other treatment options , 135 (45.5%) treated with Loop diuretics, 185(62.3%) with 5% Dextrose-0.9% Saline , 107(36%) with Sodium Bicarbonate , 48(16.2%) with Dopamine and 105(35.4%) with Haemodialysis. The outcomes of the study were 13(4.4%) progressed to CKD,192 (64.6%) recovered and 92(30.97%)Died. Chi-square test was performed and relationship was found between  treatment with 5% Dextrose-0.9% Saline and outcomes X(2,N=297) =12.58, p<.01, with Sodium Bicarbonate and outcomes X(2,N=297) =6.95, p<.05, and with Dopamine and outcomes X(2,N=297) =52.24, p<.001, no significant relationship was found in  outcomes in patients receiving  0.9% Normal Saline , Loop diuretics and Haemodialysis. CONCLUSIONS: This study reveals that there is statistical significant relationship with outcomes of AKI when treated with 5% Dextrose-0.9% Saline, Sodium Bicarbonate and Dopamine.We also conclude that, type of IV fluid administered, does not have clinically significant influence on outcomes of AKI.
