OBJECTIVES: To assess the drug related problems of In-patient settings by reviewing the patient medical chart. METHODS: The study was carried out in Medicine ward of tertiary care Hospital in South India. The study was approved by institutional ethical committee. During routine ward rounds, patients medical chart from day one to till discharge were followed. Patients with chronic disorders and polypharmacy (≥ 4 drugs) were included for this study. Medication orders were reviewed for assessment of drug related problems like dosage regimen, drug-drug interaction, adverse drug reactions and medication error. RESULTS: A total of 100 patients prescriptions were reviewed during the study period. Among this 62% were males and mean age of patients were 63.89 ±12.60 years. We found 42 drug related problems in 28 patients. Among them 40 interventions were accepted by physician and appropriate changes were made accordingly. This includes 12 dosage adjustments, 5 ADR, 12 Medication errors and 11 Drug- Drug Interaction. CONCLUSIONS: Our study concludes that the participation of clinical pharmacist in reviewing the medication order of the In-patient will enhance the medication safety by averting drug related problelms.
