OBJECTIVES: To investigate time and impact factor (IF) implications of each additional resubmission of real-world/health-economic manuscripts. METHODS: Data were derived from industry-sponsored real-world/health-economic manuscript submissions with complete submission history, as captured in our company’s project administration records. For each submission, we analyzed the time from submission to decision and from decision to resubmission, and the change in IF. RESULTS: Half of the manuscripts (21/41) were accepted at 1st submission, 13/41 at 2nd submission, 6/41 at 3rd submission, and 1/41 at 4th submission. The cumulative median (interquartile range [IQR]) time to acceptance increased with each resubmission: 106 (64–142) days for manuscripts accepted at 1st submission, 189 (91–204) days for those accepted at 2nd submission, 230 (164–347) days for those accepted at 3rd submission, and 151 (151–151) days for the one manuscript accepted at 4th submission. Manuscripts accepted at the 2nd, 3rd, or 4th submission had a lower IF compared with those accepted at 1st submission (−2.917, −4.854, and −0.913 for 2nd, 3rd, and 4th submission, respectively). For manuscripts rejected at 1st submission, the median (IQR) time from submission to decision was 14 (6–33) days, and time from decision to resubmission to the 2nd journal was a further 40 (18–64) days. When considering only the manuscripts that were rejected quickly (within 14 days) at 1st submission (10/20), the median (IQR) time from decision to resubmission was 24 (14–54) days. CONCLUSIONS: We quantified the potential delay to final acceptance and change in IF with each subsequent resubmission of real-world/health-economic manuscripts. Authors could take these data into consideration when selecting their target journal, to reduce the number of resubmissions and reduce delays to acceptance.
