ECONOMIC IMPACT OF HEALTH CARE RESOURCE UTILIZATION PATTERN AMONG PATIENTS DIAGNOSED WITH CERVICAL DYSPLASIA AND CERVICAL CANCER IN COSTA RICA

OBJECTIVES: The incidence of cervical cancer (CC) among women in Costa Rica (CR) is 6.8%. The aim of this study was to describe the patterns of healthcare resource utilization (HCRU) and associated costs for cervical dysplasia (CD) and CC in CR. METHODS: Descriptive study utilizing a Delphi panel to build consensus on patterns of HCRU in patients with CD and CC in CR. The panel consisted of 28 experts (11 gynecologists, 6 gynecologic-oncologist and 11 oncologists) from hospitals of the Social Security of CR. The implementation took 5 weeks, during which the panel members received and responded to a structured questionnaire during the first round or questions, received and reviewed data aggregated from the responses of all panel members in round one; and responded once more to the questionnaire during a second round. The questionnaire was based on a series of questions about the resource use in the treatment of cervical intraepithelial neoplasia (CIN) 1, 2, and 3 and CC; taking into account the demographic data, diagnosis tests, treatment options, clinical guidelines, adverse events and complications of the current treatments, and number of outpatient visits. The questionnaire was derived from previously published studies using a similar methodology. The costs in local currency (2015) were obtained from the official database of the Social Security of CR and converted to US dollars. RESULTS: Regarding CD, the total cost per episode of CIN-1 was US$710.33, for CIN-2 was US$1,471.54, and for CIN-3 was US$1,860.34. The cost per episode varied according to the location of the cancer: local cervical carcinoma- US$6,791.58; regional cervical carcinoma- US$9,686.68; and distant cervical carcinoma- US$9,589.50. CONCLUSIONS: The economic burden of CD and CC is relevant for CR. These findings underscore the importance of implementing public health initiatives, such as screening, education, and vaccination, for the prevention of these diseases.
