ECONOMIC AND CLINICAL EVALUATION OF ELECTROSTIMULATION IN MEDICAL REHABILITATION

OBJECTIVES: The study evaluates the effectiveness of electrostimulation in medical rehabilitation for two interventions: functional electrical stimulation for so-called drop foot syndrome, and electrostimulation of denervated muscles in spinal cord lesions. The study aim was to evaluate the cost and clinical data to create the basis of evidence for negotiations on remuneration of the electrical stimulation devices with health insurance companies. METHODS: Data were collected prospectively in the Rehabilitation Centre Kladruby and the University Hospital Královské Vinohrady. Czech market of devices for electrostimulation and functional electrical stimulation was analyzed. The effect of quality of life and some other parameters on the therapy were measured using two questionnaires (EQ-5D + an additional questionnaire compiled for the particular use in electrostimulation). Selected devices were evaluated using the methods of multi-criteria decision making. Direct medical costs of electrostimulation and cost-effectiveness analysis were calculated. RESULTS: Generally, a positive influence of electrostimulation therapy to patient quality of life was observed. Especially for drop foot syndrome treatment, the effect was more significant along with improving gait parameters. The highest cost item was personnel costs for both types of treatment. In the case of functional electrical stimulation only two devices used in the Czech Republic were evaluated; in the case of electrostimulation of denervated muscles, 11 most frequent devices out of 35 generally used were analyzed. Despite slightly higher costs of electrostimulation, the resulting values of ICER are in a reasonable cost-effectiveness range. CONCLUSIONS: The results are in agreement with findings of other authors that electrostimulation increase the speed of reinnervation process and its quality. The effect of the therapy as compared with comparators (ankle-foot orthosis for functional electrical stimulation; therapy alone for electrostimulation of denervated muscles) is not significantly higher in all dimensions, but the survey shows patient preference for the use of these devices.
