OBJECTIVES: A literature review of cost-effectiveness (CE) analyses identified that the CE model outcomes were sensitive to estimates of quality-adjusted life year (QALY) losses due to herpes zoster (HZ) and postherpetic neuralgia (PHN). This study’s objective was to assess UK-specific QALY losses associated with HZ and PHN in immunocompromised (IC) and IC-free individuals aged ≥50 years of age (YOA).

METHODS: We conducted a systematic literature review of utility estimates related to HZ and PHN by level of pain severity and estimated UK-specific severity weighted utility values and utility losses accounting for age-specific baseline values. We calculated the overall QALY loss for a HZ only case, i.e. not involving PHN, accounting for the acute (days 0 to 30) and subacute (days 30 to 90) periods and the QALY loss for a PHN subject using the mean duration of PHN.

RESULTS: The utility losses related to HZ and PHN were estimated to be 0.173 and 0.211, respectively, for all subjects. The QALY loss associated with HZ only ranged from 0.022 to 0.024 for IC-free subjects aged 50 to 59, and ≥80 YOA, respectively, and from 0.023 to 0.025 for IC subjects aged 50 to 59, and ≥80 YOA, respectively. Corresponding values for PHN, QALY losses were 0.181 to 0.227 for IC-free subjects and from 0.253 to 0.317 for IC subjects. The overall QALY loss associated with HZ and PHN together ranged from 0.036 to 0.062 for IC-free subjects aged 50 to 59, and ≥80 YOA, respectively, and from 0.063 to 0.119 for IC subjects aged 50 to 59, and ≥80 YOA, respectively.

CONCLUSIONS: Higher overall QALY losses were estimated for IC subjects compared to IC-free subjects. This was influenced primarily by a higher percentage of IC subjects experiencing PHN and a longer duration of PHN in these subjects.
