HEALTH ECONOMIC MODEL FOR ESTIMATING THE DIRECT COST IMPACT OF ADOPTING A VASCULAR ACCESS ANTIMICROBIAL BUNDLE OF SOLUTIONS IN ICU SETTINGS IN GERMANY

OBJECTIVES : Develop a health economic budget impact model to estimate the direct medical cost savings of replacing traditionally used non-antimicrobial dressings and catheter port caps with 3M vascular access antimicrobial bundle consisting of CHG-dressing and disinfecting catheter port caps. METHODS : A static decision tree model was used to estimate the budget impact of 3M vascular access bundle from a hospital payer perspective. The model time horizon is one year. Epidemiological model parameters included CLABSI rate per 1000 catheter days – 1.48 and risk of local site infection per patient – 14%. Estimated efficacy of 3M vascular access bundle is assumed to be 97.3% reduction of CLABSI, by targeting both extraluminal and intraluminal routes of infections and 63.3% reduction of local site infection. The direct medical costs are based on a daily cost of inpatient stay in ICU of Eur 1,133 reported for Germany and average length of stay attributable to CLABSI of 9.33 days. An alternative micro costing method was considered. All model data is based on published literature. Parametric uncertainty was evaluated with a one-way sensitivity analyses. RESULTS : In a hypothetical ICU consisting respectively of 10 beds the annual total savings were Eur 38,203 (61% decrease) when 3M vascular access antimicrobial bundle is compared to a current practice.. When alternative micro costing method was considered the total savings were Eur 64,273 (71% decrease). The number of CLABSI and local site infections were reduced from 4.9 and 76.9 to 0.1 and 28.2 respectively. Rate of CLABSI and intervention efficacy were the most influential parameters identified in the one-way sensitivity analysis. CONCLUSIONS :
